About the role
The Regional Practice Manager, Patient Care Coordination (RPM) owns the day-to-day performance of the Patient Care Coordination (PCC) team within their assigned region. Working through a direct-report Lead, the RPM ensures the team consistently meets key performance indicators, delivers high-quality patient support, and runs efficient, reliable workflows. The RPM also partners closely with providers on key clinical-adjacent functions — intake, scheduling, documentation readiness, and follow-up — so that every patient and provider is set up for a successful visit.
Responsibilities
- Team Leadership & Performance
- Oversee the daily operations of the PCC team through direct management of the PCC Lead, ensuring clear priorities, workload balance, and accountability.
- Own regional KPIs for the PCC function (e.g., response times, scheduling turnaround, coordination accuracy, patient satisfaction) and drive corrective action when targets are missed.
- Coach, develop, and support the Lead in managing coordinator performance, including 1:1s, feedback, and performance improvement plans.
- Monitor staffing needs, capacity, and coverage across the region to ensure consistent service levels.
- Serve as an escalation point for complex or sensitive patient issues, ensuring timely and empathetic resolution.
- Champion a patient-first culture, ensuring every interaction reflects Form Health's standards of care.
- Workflow & Process Improvement
- Continuously evaluate PCC workflows to identify bottlenecks, inefficiencies, and opportunities for improvement.
- Design, test, and roll out process changes and standard operating procedures in partnership with Clinical, Product and Technology partners.
- Document and maintain up-to-date workflow guides, job aids, and training materials for the PCC team.
- Use data and reporting tools to track performance trends and inform operational decisions.
- Provider & Clinical Support
- Partner with providers to support key clinical-adjacent functions, including patient intake, pre-visit preparation, scheduling, and post-visit follow-up, to set both patients and providers up for successful visits.
- Act as a liaison between providers and the PCC team, ensuring provider needs and feedback are understood and addressed.
- Identify and resolve recurring friction points between clinical and coordination workflows.
Requirements
- At least 3-5 years of experience in virtual healthcare operations, patient care coordination, practice management, or a related clinical support function, including experience managing or leading a team.
- Bachelor's Degree in Healthcare Administration or related field preferred; active MA Certification a plus.
- Demonstrated success managing to KPIs and driving measurable improvements in team performance or quality.
- Strong understanding of clinical workflows, patient intake/scheduling processes, and the patient support lifecycle.
- Excellent people leadership skills, with experience coaching and developing supervisors or leads.
- Exceptional communication skills, with the ability to work cross-functionally with clinical providers, product and technology partners.
- Comfortable working with data and reporting tools to monitor performance and identify trends.

$70,000—$95,000 USD base annual salary
