As AdviniaCare's Regional Clinical Liaison for Rhode Island, you'll be the bridge between hospitals, hospice agencies, community organizations, and our facilities. You'll meet patients where they are, assess their needs, and help them and their families find a program where they'll thrive. This role is ideal for a clinically sharp, people-focused nurse who enjoys working independently, building relationships, and spending the day out in the community rather than on a unit. What You'll Do Respond promptly to referrals from hospitals, hospice agencies, and community social service partnersMonitor electronic referral platforms for qualified admissions in your coverage areaConduct on-site clinical evaluations to determine whether a patient is a good fit for our programsShare completed evaluations with the Admissions Director, along with a verbal overview of the patient's conditionEducate patients and families about our programs, answer their questions, and arrange facility tours with our Admissions CoordinatorOccasionally screen local hospital referrals for AdviniaCare facilities outside Rhode IslandBuild and strengthen relationships with referral sources, community contacts, and facility teamsCollaborate with Department Heads and nursing and rehab staffTrack referrals by hospital, participate in daily and weekly census calls, and provide reports as neededSupport marketing and outreach efforts What You Bring Active Rhode Island LPN or RN license (required)Previous clinical liaison, admissions, or referral experience strongly preferredStrong communication and interpersonal skills, with the ability to make families feel at easeA professional, positive presence as the face of AdviniaCare in the communitySelf-motivation and solid organizational skills for managing your own schedule and territory Ready to make a difference?
