Clinical Liaison (RN/LPN/Therapist)

Join an exceptional team to navigate the full post-acute care continuum. The Clinical Liaison will play an integral role in our team by ensuring care continuity of patients and promoting our care continuum.

Responsibilities

• Ensure care continuity of patients and promote the care continuum.
• Manage and participate in Marketing and Admissions Intake activity with minimal direction.
• Achieve results by designing and achieving appropriate Marketing/Admissions Intake goals and objectives.
• Tailor marketing strategies to network with referral sources and develop a marketing plan for the hospital.

Requirements

• Clinical discipline with a current license in good standing.
• At least 2 years of clinical experience.
• Post-acute care experience preferred.
• Working knowledge of computers and software packages such as Microsoft Office.
• Knowledge of hospital databases.
• Proven ability to work with physicians, social workers, insurance companies, case managers, patients, and families.
• Current valid LA Driver’s License.
