Job Overview

Process incoming and outgoing client referrals in a timely manner.
Coordinate with the agencies to ensure clients receive appropriate services.
Verify client eligibility, insurance, and necessary documentation for services.
Maintain accurate and up-to-date records in the referral management system.
Communicate effectively with clients to schedule appointments and provide necessary information.
Follow up on referrals to ensure successful service connection and client engagement.
Identify barriers to care and assist clients in overcoming obstacles to accessing services.
Adhere to HIPAA and confidentiality guidelines.

Qualifications

  • A high school diploma or equivalent is required.
  • An associate or bachelor’s degree in a related field is preferred.
  • Experience in behavioral health, mental health, healthcare administration, or a similar field is a plus.
  • Strong organizational and multitasking skills are essential.
  • Excellent communication and interpersonal abilities are required.
  • Familiarity with electronic health records (EHR) and referral management systems is preferred.
  • Understanding of insurance verification and community resources is a plus.
