Referral Navigator

About the role

The Patient Navigator Clinical identifies and implements nursing interventions and evaluates results for a wide variety of patient populations, directs and acts as a resource to other members of the nursing team as assigned and promotes compliance and support for policies.

Requirements

• Valid California Registered Nurse License without restriction
• American Heart Association (AHA) or American Red Cross Basic Life Support (BLS), or UC Davis Health CPR
• Verification of graduation from an accredited school of nursing
• Minimum of two (2) years as an ambulatory RN
• Recent Sports Medicine/Orthopedics new patient referral RN triage experience
• Knowledge of current trends in nursing
• Commitment to excellence in patient care
• Demonstrates accountability and professionalism
• Adapts to change in a positive, professional and productive manner
• Knowledge of legal implications in clinical practice and documentation
• Knowledge of performance improvement and evidence-based practice
• Advanced computer skills

$9,283.33 - $19,150.00

Schedule

Mon- Fri 8am-5pm
