Referral Coordinator
Overview
BJC Home Care offers a wide range of home care services including skilled nursing, hospice, rehabilitation therapy, and more. Located in Missouri and Illinois, BJC Home Care serves over 25 counties and is the largest home care network in the region.
Role Purpose
The Intake Coordinator (HCS) processes home care referrals accurately and efficiently, ensuring compliance with agency policies and admission criteria. They serve as a resource liaison for all product lines and promote customer satisfaction.
Responsibilities
- Screens referrals to ensure patients meet agency admission criteria and comply with state and federal regulations.
- Verifies insurance benefits, obtains pre-authorizations as necessary, assists billing department, and interprets company services, admission criteria, and medical policies to referral sources and individuals seeking information.
- Tracks current clients that are either hospitalized, placed in a short-term skilled facility, or in a rehabilitation facility.
- Communicates and coordinates discharge planning; educates internal and external customers about Home Care Services and/or outpatient services; coordinates pre-scheduling with BJC Home Care service lines and assures patient has all necessary post-acute care or non-acute care.
Minimum Requirements
- High School Diploma or GED
- 2-5 years of experience
- Preferred: Associate's Degree
- Preferred: 5-10 years of experience, including supervisor experience
