The Recovery Support Navigator
$1000 Bonus Offered!
- Location: Framingham, MA
- Schedule: Monday - Friday, 8:00am-4:00pm
- Pay Rate: $22.90-$26.51/hour

Responsibilities
- Provide clinical case management to engage, educate and support patients seeking treatment upon release and their families
- Motivate and maintain patients in culturally relevant behavioral health services in the community
- Engagement begins 120 days pre-release, support continues up to 90 days post-release to the community
- Work independently and as a team
- Avoiding transportation and housing resources and make referrals as needed
- Coordinating necessary treatment for various patient needs
- Communicating with all referral sources and treatment entities using proper releases of information
- Working hours required to meet the business needs of the organization
- Travel to other programs or locations as needed
Qualifications
- High school diploma required and LADC-II preferred
- Bachelor’s degree in human services or related field or equivalent experience in the behavioral health field preferred
- Case management experience strongly preferred
