Location: Haverhill, MA

Outpatient Treatment Program Schedule: 5:30pm - 2:00pm, Monday- Friday

Pay Rate: $22.90-$26.51

Responsibilities

• Providing quality services and case management to assigned patients in accordance with the Recovery Navigator model.
• Performing intake assessments on all new patients to the program and completes all associated documentation within required time frames.
• Working with the patient to develop and implement an individualized plan with goals and objectives based on identified needs.
• Aiding patients with scheduling and coordinating medical and behavioral health appointments.
• Cooking care and the plan with existing providers including but not limited to behavioral health, primary care and Community Partner.

Qualifications

• Bachelor’s Degree in human services related field or discipline
• LADC (Licensed Alcohol and Drug Counselor)
• Recognized certification and/or licensure for the target population to work with
• Sufficient experience and training in counseling and meets regulatory requirements
