Essential Duties And Responsibilities
- Perform work associated with recovery support services, therapeutic care, case management, treatment and rehabilitation of persons with drug and/or alcohol use through care coordination.
- Collaborate with team members including: patient; primary care provider; Behavioral Health provider and others to deliver comprehensive care to patients of Family First Health.
- Engage in brief relationship building and active listening with patients receiving treatment.
- Provide supportive (peer-based) coaching and encouragement to patients contemplating or actively seeking help with substance use treatment and recovery.
- Aid in the development of comprehensive, individualized recovery plans incorporating resources that support patient goals and interests.
- Serve on-call duties based on on-call policy and departmental procedures.
- Provide hope and encouragement needed for the client in reaching his/her goal(s) for recovery.
- Connect with other providers.
- Identify, develop, and maintain positive linkages with agencies and social service organizations/services in the community and within FFH.
- Facilitate warm handoffs between all available behavioral health and physical/dental health treatments.
- Create and manage long-term individualized recovery plan by assessing patient’s target needs and reassess on timely basis, including making appropriate referrals for treatment and rehabilitation of drug and alcohol dependent patients.
- Attend mandatory treatment team/clinical round meetings to discuss the patient’s recovery process while in treatment including stabilization, behavior modification, MAT options, discharge and aftercare planning.
- Maintain timely and accurate documentation and tracking of all service provision, evaluation, and statistical data for the program; including detailed patient demographics, medical information, risk factors, and other information needed related to grant funding.
Knowledge/Skills/Abilities
- Minimum of two (2) years uninterrupted recovery.
- Minimum of two (2) years of experience in a recovery-oriented setting.
- Must have or be eligible to pursue Certified Recovery Specialist certification (CRS).
- Previous education/experience with drug and alcohol programs preferred.
- Excellent customer service skills with the ability to effectively establish appropriate boundaries.
- Strong written and verbal communication skills with the ability to support individuals of all backgrounds.
- Displays critical thinking and exercise individual judgement and discretion in matters of significance.
- Ability to establish and maintain effective working relationships with patients, employees, volunteers, and the public.
- Ability to be organized and work efficiently with strong attention to detail within a demanding work environment while prioritizing tasks.
- Thorough knowledge of the Guiding Principles of Recovery.
- Knowledge of MAT and ability to perform duties in the context of MAT.
- Knowledge of computer software applications (Microsoft Office Applications, others).
- Knowledge of grammar, spelling, and punctuation as it relates to correspondence necessary to perform essential job functions.
Physical Demands
- Frequent and prolonged work at a computer and/or on the phone for extended periods of time with repetitive typing with arm and hand motion.
- Frequent travel between work sites with exposure to weather conditions.
- Requires correct vision and hearing to normal range.
- Frequent bending, stooping, crouching, or kneeling.
- Frequent standing, walking, turning, and reaching.
- Frequent contact with patients, clients, vendors, and visitors.
- Occasional carrying of office supplies and lite equipment.
- Frequent competing demands, frequent changes, delays, or unexpected events.
