RAC Resident assessment coordinator(MDS)

Join Armstrong Rehabilitation & Nursing Center, Your Home Away from Home. As a RAC Resident Assessment Coordinator, you will play a crucial role in ensuring high-quality care for our residents while maintaining regulatory compliance.

Responsibilities

• Conduct comprehensive assessments of residents to identify their medical, social, and psychological needs
• Develop and implement individualized care plans that address residents' unique needs and goals
• Cookordinate with healthcare providers, including physicians, therapists, and social workers, to ensure comprehensive care
• Ensure seamless transitions between care settings, including hospital, rehabilitation, and long-term care
• Monitor residents' progress and adjust care plans as necessary
• Maintain accurate and detailed records of assessments, care plans, and resident outcomes
• Participate in quality improvement initiatives to ensure high-quality care and regulatory compliance

Requirements

• 1+ years of experience in resident assessment, nursing, or a related field
• Strong understanding of Medicare and Medicaid regulations, including RAI and MDS
• Excellent communication, interpersonal, and organizational skills
• Able to work effectively in a fast-paced environment with multiple priorities
• Strong analytical and problem-solving skills
