Role Summary
Country Lane Manor is seeking a compassionate and detail-oriented Registered Nurse Assessment Coordinator (RNAC / MDS Coordinator) to join their skilled nursing team in Keosauqua, Iowa.
Responsibilities
- Conduct routine resident assessments according to established schedules and as needed for changes in condition, hospital readmissions, and other clinical events.
- Accurately observe, assess, and communicate changes in resident condition to the interdisciplinary care team.
- Collaborate with Social Services, Activities, Dietary, Therapy, and Nursing to ensure timely completion of assessments, CAAs, and supporting documentation.
- Complete MDS assessments, quarterly reviews, CAAs, and care plans in compliance with state and federal regulations.
- Develop, update, and monitor individualized resident care plans based on assessment findings.
- Prepare and distribute the MDS schedule to the interdisciplinary team.
- Transmit MDS data to the state as required and maintain accurate resident assessment records.
- Participate in a rotating on-call schedule every 3–4 weeks, including weekend on-call responsibilities.
Qualifications
- Current Registered Nurse (RN) license in the State of Iowa.
- Minimum of one (1) year of MDS Coordinator or MDS experience in a skilled nursing or long-term care setting.
- Strong knowledge of MDS processes, RAI guidelines, assessment schedules, CAAs, and care planning requirements.
- Excellent organizational, communication, and collaboration skills.
- Ability to work independently while partnering effectively with the interdisciplinary team.
