We are an agency committed to quality gender-responsive, trauma-informed care to individuals experiencing serious mental illness, development disabilities, and co-occurring disorders. Metrocare programs focus on the issues that matter most in the lives of the children, families and adults we serve. The Housing QMHP provides supportive housing case management services to individuals with mental illnesses who have experienced homelessness or instability in housing. Services may include, but are not limited to, assessment, development of measurable treatment goals and objectives, referral, linkage, advocacy, monitoring, crisis intervention, life skills training, tenancy skills training, and continuity of care. This position promotes ongoing housing stability, promotes the acquisition of income to increase self-sufficiency, and encourages self-determination of program participants.

Responsibilities

• Provides overall service coordination, referral, linkage, and advocacy to consumers with varying needs.
• Performs duties in concert with other members of an interdisciplinary treatment team.
• Completes ongoing psychosocial assessments for program participants.
• Upon assessment, formulates, collaboratively an individual housing service plan.
• Develops measurable objectives and goals agreed upon by the program participant.
• Assists in obtaining and modifying goals as needed.
• Provides skills training to promote stability in housing (budgeting, self-care, developing natural supports, and independent/instrumental living skills).
