Key Responsibilities
- Provide strengths-based training utilizing an evidence-based curriculum.
- Work with Case Manager to provide services: Skill Training & Development, Medication Training & Support Services, and Crisis Intervention Services.
- Implement the Service/Treatment Plan focusing on client needs and recognizing their strengths.
- Document all client interactions and maintain records according to regulatory/agency standards.
- Establish rapport with clients, family members, and other individuals close to the client; provide emotional support and encourage engagement in the service process.
- Maintain records and progress notes in the electronic record system.
- Assist clients with daily living tasks such as behavior management, counseling services, budget management, independent transportation assistance, time management skills, and other tasks to promote independent living.
- Collaborate with court/probation, tribal/community agencies, families, and other natural support systems to ensure clients' needs are met.
- Develop a strong working relationship with community partners.
- Assist clients in practicing effective communication, problem-solving, and anger management techniques in their daily lives.
- Participate in staff meetings, monthly training, company programs, and professional development training as required.
Qualifications
- Applicants must have a Bachelor's Degree in Human Services applying for QIDP.
- Applicants must have a Master's Degree in Human Services applying for Case Manager.
- All applicants must have at least (1) year of paid experience.
