Key Responsibilities

  • Provide strengths-based training utilizing an evidence-based curriculum.
  • Work with Case Manager to provide services: Skill Training & Development, Medication Training & Support Services, and Crisis Intervention Services.
  • Implement the Service/Treatment Plan focusing on client needs and recognizing their strengths.
  • Document all client interactions and maintain records according to regulatory/agency standards.
  • Establish rapport with clients, family members, and other individuals close to the client; provide emotional support and encourage engagement in the service process.
  • Maintain records and progress notes in the electronic record system.
  • Assist clients with daily living tasks such as behavior management, counseling services, budget management, independent transportation assistance, time management skills, and other tasks to promote independent living.
  • Collaborate with court/probation, tribal/community agencies, families, and other natural support systems to ensure clients' needs are met.
  • Develop a strong working relationship with community partners.
  • Assist clients in practicing effective communication, problem-solving, and anger management techniques in their daily lives.
  • Participate in staff meetings, monthly training, company programs, and professional development training as required.

Qualifications

  • Applicants must have a Bachelor's Degree in Human Services applying for QIDP.
  • Applicants must have a Master's Degree in Human Services applying for Case Manager.
  • All applicants must have at least (1) year of paid experience.
