QMHP Access & Diversion- Intensive Services
About the role
This Access and Diversion Qualified Mental Health Professional (QMHP) Community Services (CS) reports directly to the Access and Diversion Supervisor within the behavioral health division. The position serves individuals who present to LifePath Systems Diversion Center by performing face-to-face crisis assessments, crisis intervention, crisis follow-up, relapse prevention services, intake assessments, recovery and safety planning, resource navigation, referrals to appropriate community services, and connection to outpatient behavioral health services.
Key responsibilities include ensuring timely access to services, collaborating with the LifePath Systems team and community organizations to identify needs and reduce barriers, and improving social determinants of health. The role is cross-trained with the Crisis Center to assess crisis resolution, stabilization, and linkage to services to reduce inpatient and law enforcement interventions.
Services are provided with a trauma-informed and person-centered approach, emphasizing service excellence, resource stewardship, and adherence to policies, contracts, and regulatory requirements. The position builds relationships with local police agencies and mental health professionals to increase referrals and demonstrates professional conduct while coordinating care with external entities.
Responsibilities
- Perform crisis assessments, crisis intervention, and follow-up services.
- Conduct intake assessments, recovery and safety planning, and resource navigation.
- Provide referrals to community services and connect individuals to outpatient behavioral health services.
- Ensure individuals receive timely access to appropriate services and reduce barriers to care.
- Collaborate with the LifePath Systems team and community organizations to improve social determinants of health.
- Assess crisis resolution and stabilization, linking individuals to services to reduce inpatient and law enforcement interventions.
- Document engagement and services in the electronic health record for at least half of the time worked.
- Sign at least 90% of service notes within 2 business days of service.
- Complete and sign all crisis documentation by the end of the shift.
- Complete assigned follow-ups at 24 hours, days 1-7, 30, 60, and 90-day intervals.
- Communicate shift reports at the end of each shift.
- Achieve a score of 80% or higher on full chart reviews following the Golden Thread.
- Provide person-directed services to assist in navigating access to medical, social, vocational, financial, educational, and other individualized services.
- Explain LifePath Systems service criteria to individuals and community partners.
- Attend monthly team meetings.
- Work center holidays if they fall on a scheduled workday (24/7 operation).
- Notify supervisor within 4 hours of regularly scheduled work time in case of unforeseen Paid Time Off needs.
- If bilingual and receiving a stipend, translate for other staff and use approved language in service provision as needed.
- Assess and de-escalate behavioral health crisis situations calmly and respectfully.
- Maintain center-issued equipment and complete required training by due dates.
- Build positive relationships with local police agencies and mental health professionals to increase referrals.

- Saturday–Sunday, 7:00 PM – 7:00 AM
- Saturday–Sunday, 7:00 AM – 7:00 PM
Qualifications
- Bachelor’s degree in psychology, social work, or a related field from a four-year college or university.
- CCBHC knowledge and experience preferred.
- Experience in Texas Recovery and Resiliency (TRR) preferred.
- Knowledge of Trauma-Informed Care Practices required.
- Reliable transportation and a valid driver’s license.
Skills
- Experience providing psychosocial assessments, treatment planning, case management, service coordination, skills training, crisis intervention, and symptom management for individuals with serious mental illness or serious emotional disturbance.
- Ability to assess individual needs, coordinate services, and communicate effectively.
- Knowledge of the social services system and ability to connect individuals with appropriate services.
- Strong written communication skills for service documentation and internal communications.
- Ability to think clearly and logically in potentially dangerous or emotional situations.
- Patience, empathy, and understanding when responding to individuals in crisis.
- Organized, detail-oriented, and computer-proficient.
- Strong multicultural competency.
- Bilingual (English/Spanish) preferred.
