QIDP Case Manager Home Based Services - Rose Center

About the role

Envision Unlimited is a non-profit agency dedicated to serving adults with intellectual/developmental disabilities and/or mental illness. We focus on providing quality services that promote independence and community integration through person-centered care.

Responsibilities

• Coordinate the completion of service agreements and service authorizations.
• Work with families to develop the Employer of Record Implementation Strategy.
• Maintain client files in the Quantum electronic records system.
• Complete other reports/forms as assigned.
• Seek out and effect linkage with referral sources in the community when appropriate.
• Collaborate with other staff and programs regarding the needs of shared members.
• Accompany members to referral services when indicated and act as an advocate.
• Aid the supervisor in special projects.
• Meet weekly with the supervisor to discuss cases.
• Maintain an accurate work schedule and mileage records.
• Perform other related responsibilities as assigned.

Qualifications

• Bachelor’s degree in human or social service field.
• At least one year of direct experience working with adults with DD.
• Must be QIDP (Qualified Intellectual Disability Professional) eligible upon hire.
• Valid driver’s license.
• Willingness to travel up to 70% of the time and work from the Rose Center office (gas and mileage reimbursement provided).

Schedule

Generally Monday-Friday 9am-5pm with flexibility when needed. This job requires the candidate to travel to individuals' homes throughout the Chicagoland (Cook County area).
