In-Home Services Case Manager
About the role
Saint Luke’s Crittenton Children’s Center in South Kansas City, MO is seeking a remote In Home Services Case Manager to join their Pediatric Behavioral Health Team. This position manages a caseload of clients aged 4 to 18 in partnership with Beacon Mental Health, serving Clay, Ray, and Platte counties. The role provides in-home clinical case management to pediatric behavioral health patients and their families, seeing 12–15 families per week and coordinating care from treatment planning through discharge to help patients remain in their homes rather than hospitalized care. This is a remote position with occasional on-site team meetings; services are delivered in patients' homes and community settings across the Northland counties.
Responsibilities
- Provide clinical case management services to children and families in community settings
- Assist with implementing treatment plans developed by master-level clinicians
- Develop crisis plans
- Develop strategies for child success in school, home, and community
- Teach behavioral interventions to parents/guardians, schools, and other stakeholders
- Connect children and families to community resources
- Apply a "wrap-around" philosophy to provide family resources including respite services
- Provide 24/7 on-call availability
- Partner with care providers, schools, families, and outside services to coordinate and support patients
- Provide a full range of services including clinical case management, education and coaching, skill building, crisis intervention, and consultation with community agencies
Requirements
- Bachelor's degree in Social Work, Psychology, Counseling, or related field
- 1 year of applicable experience
- Valid driver's license with clean driving record
- Applicable vehicle insurance
Schedule
Full-time, Monday–Friday swing shift.
