QIDP/ Case Manager (Community Options)

This position follows Rule 50, Dept of Human Services Rule 115, Rule 116, Rule 119, Rule 120, and other relevant regulatory guidelines.

About the Role

The Support Services Director will develop and implement personal plans and coordinate all direct care services provided by the facility to residents. This role supports the agency’s mission, vision, values, and strategic plan, ensuring all responsibilities align with individual choice and appropriate policies, procedures, and regulations.

Responsibilities

• In conjunction with the HR department, interview, hire, supervise, assign, evaluate the performance of, and when necessary, discharge assigned personnel directly and in cooperation with appropriate department heads.
• Coordinate and monitor functions of each area of facilities to ensure efficient operation, including maintenance and upkeep of buildings, grounds, and vehicles (if used).
• Meet all deadlines set by the agency and outside resources.
• Work with the Benefits Coordinator and complete monthly summaries, ensuring all documentation meets clinical and financial requirements.
• Maintain files and data entry according to agency standards.
• Provide support to and collaborate with all team and agency members.
• Develop personal plans for all individuals on caseload, including coordinating and facilitating ISP annual meetings and other relevant meetings.
• Train staff in clinical responsibilities, including incident reports, ISPs, monthly summaries, drill sign-offs, and other clinical tasks.
• Train staff in Positive Behavior Approaches related to individuals and develop behavior plans for those requiring behavioral support.
• Participate in Behavioral Management and Human Rights Committees as needed.
• Maintain positive parent and guardian relations for individuals on the team.
• Promote community-based learning for individuals and understand rules governing program/licensure for community day services.
• Support database management systems and complete QIDP training per DHS regulations.
• Monitor inventories and purchasing systems, review purchasing inventories, and reconcile with current invoices.
• Complete and follow accounting and payroll procedures in a timely manner.
• Perform other duties as assigned and act as a mandated reporter for all allegations of abuse and neglect.

Requirements

• Bachelor’s Degree preferred in Human Services or education; two years of supervisory experience required.
• Supervisory experience is essential.
• One year of experience with persons with developmental disabilities.
• Ability to read and comprehend simple instructions, short correspondence, and memos.
• Ability to write correspondence and effectively present information in one-on-one and small group situations.

Physical Requirements

• Frequent walking and sitting; regular reaching, grasping, and standing.
• Other physical activities related to an office environment.
• Operates general office equipment, including multi-line phone, fax machine, printer, copier, and personal computer.
• Ability to lift 50 lbs.
