About the Role
The case manager will serve individuals with behavioral health needs, gather information, and establish eligibility for various community resources. Frequent travel to individuals' homes, schools, and community settings is required. The position may require working outside of normal business hours to provide a full array of behavioral health services and meet program needs.
Responsibilities
- Provide case management, skills training, medication training and support, crisis interventions, and psychosocial rehabilitation services using a trauma-informed and person-centered approach.
- Engage with individuals served and deliver exceptional customer service, documented in the electronic health record (minimum 80 hours per month).
- Maintain work quality and internal/external fidelity audits at or above 90%.
- Practice good stewardship of resources by completing required trainings prior to due dates and maintaining center-issued equipment.
- Provide person-directed case management to assist in navigating access to medical, social, vocational, financial, educational, and other individualized services.
- Ensure adherence to the Health and Human Services Commission, Texas Administrative Code, contracts, grants, and Lifepath Systems policies and procedures.
- Document services in the electronic health record within 2 business days at a rate of 90% or above.
- Build positive relationships with community-facing agencies and maintain professional relationships with all stakeholders and partners.
- Promote choice and autonomy through a strength-based model to improve access to social determinants of health.
- Work independently and collaboratively to maintain appropriate boundaries and foster better outcomes for individuals served and community partners.
- Assess and de-escalate behavioral health crisis situations in a calm and respectful manner.
- If bilingual (Spanish/English) and receiving a stipend, translate for other staff as needed and utilize approved language in service provision.
Requirements
- Bachelor’s degree in psychology, social work, or a related field from a four-year college or university.
- At least two years of experience in working with the community (e.g., social services, recovery, customer service, sales).
- Bilingual (Spanish/English) preferred.
- Reliable transportation and a valid driver’s license.
Skills
- Knowledge and experience in providing case management, skills training, and symptom management services to individuals with mental illness.
- Ability to assess individual needs, coordinate services, and communicate effectively.
- Knowledge of the social services system and ability to connect individuals with appropriate services.
- Strong written and professional communication skills.
- Ability to think clearly and logically to handle potentially dangerous and/or emotional situations.
