PT Weekend Track Nights LPN- Hawthorne Ridge- Assisted Living

Part-time night shift position on a weekend track (12-hour shifts every other weekend, Saturday and Sunday, 7:00 PM – 7:00 AM).

About the Role

As a Licensed Practical Nurse (LPN) at Hawthorne Ridge, you'll play a vital role in enhancing the lives of our residents. This position provides care in a residential setting designed for individuals with memory loss, Alzheimer’s disease, and other related dementias. Eddy Hawthorne Ridge is a 66-bed assisted living community on 14 beautiful acres in East Greenbush, New York.

LPNs are responsible for medications, treatments, and staff oversight under the direction of the RN. You will bring trust, comfort, and empathy to all resident interactions, making a positive difference in their lives.

Responsibilities

• Supports the Trinity/SPHP Vision, Mission, and Values in all encounters with residents and families.
• Adheres to the SPHP Code of Conduct and demonstrates accountability as an individual and team member.
• Provides direct and indirect care under the direction of an RN, including procedures, treatments, and related duties within the NYS Scope of Practice for an LPN.
• Delivers compassionate, person-centered care while fostering dignity and independence for each resident.
• Conducts daily medication administration with accuracy in accordance with physician orders and facility guidelines.
• Provides leadership for the care team during your shift, including guidance and support.
• Collaborates closely with resident assistants, other nurses, and management in a cooperative, respectful environment.
• Completes required documentation promptly and accurately to support resident care.
• Consults with the RN on any changes in a care plan or schedule and communicates with residents and their families about their care plan.
• Attends and actively participates in in-servicing, orienting, and educational activities.

Requirements

• Must be a high school graduate.
• Must graduate from a nursing education program acceptable to the New York State Education Department (NYSED).
• Must be licensed with current registration in New York.
• Ability to stand, walk, and lift for long periods of time.
• Ability to read and write using medical terminology.
• Working knowledge of computers, including electronic medication administration records, processing orders, checking email, and completing required in-servicing.
• Visual and auditory acuity adequate to perform observations and patient care.
• Maintain composure and positivity in a high-stress environment.
• Promote physical, cognitive, and psychosocial well-being of each resident.
• Work with residents in a highly personal manner during stages of acute and chronic illness.

Pay

Pay range: $24.00–$30.10 per hour. This position is eligible for shift differentials of $4.00 per hour on weeknights and $8.00 per hour on weekends. Pay is based on experience, skills, and education.
