Position Summary
The Licensed Practical Nurse (LPN) plays a vital role in enhancing the lives of residents at Hawthorne Ridge, providing assistance and care in a residential setting for individuals with memory loss, Alzheimer's disease, and other related dementias.
Principal Responsibilities
- Supports the Trinity/SPHP Vision, Mission, and Values in all encounters with residents and families.
- Adheres to the SPHP Code of Conduct.
- Demonstrates accountability as an individual and team member in meeting basic resident and family needs.
- Provides direct and indirect care under the direction of an RN, including procedures, treatments, and related duties as assigned and within the NYS Scope of Practice for an LPN.
- Delivers compassionate, person-centered care while fostering a sense of dignity and independence for each resident.
- Conducts daily medication passes with accuracy and care in accordance with physician orders and facility guidelines.
- Provides leadership for the care team during your shift, including guidance and support.
- Works closely with resident assistants, other nurses, and management in a cooperative, respectful environment.
- Completes required documentation promptly and accurately to support resident care.
- Communicates with patients and their families about their care plan.
- Attends and actively participates in in-service, orienting, and educational activities.
Education Requirements
- Must be a high school graduate.
- Must graduate from a nursing education program acceptable to the New York State Education Department (NYSED).
- Must be licensed with current registration in New York.
- Must be able to: stand, walk, and lift for long periods of time; read and write using medical terminology; have working knowledge of computers including electronic medication administration records, processing of orders, checking email, and performing required in-services, along with use of other required applications; maintain visual and auditory acuity adequate to perform observations and patient care; maintain composure and positivity in a high-stress environment; promote physical, cognitive, and psychosocial well-being of each resident; work with residents in a highly personal manner during stages of acute and chronic illness.
Pay and Benefits
The pay range for this position is $24.00-$30.10 per hour, with potential shift differentials of $4.00 per hour on weekdays and $8.00 per hour on weekends. Positions are exempt under the Fair Labor Standards Act (FLSA) and will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location. Benefits may include health insurance, retirement plans, and paid time off.
