A full-time, part-time day shift role providing hands-on care to geriatric patients in their homes. Paid training and mileage reimbursement are provided. Coverage area: Cleveland, GA. Training/Admin Office: Dahlonega.

Pay

$51.40 to $57.82 hourly, paid per visit/per point after training.

Responsibilities

  • Help patients maintain, restore, and improve movement, activity, and function.
  • Provide interventions, conduct reexaminations (every 30 days, at minimum), modify interventions as necessary, and perform discharge planning.
  • Other duties as assigned.

Requirements

  • Current license to practice physical therapy in the state of assignment.
  • Graduate from an accredited physical therapy educational program (CAPTE-accredited).
  • One year of related physical therapy experience for PTs entering home health care (or approval from regional clinical leaders with appropriate mentoring/residency participation if less than one year).
  • Current CPR certification.
