About the role

Responsibilities

  • Conduct comprehensive psychosocial assessments to identify patient and family needs
  • Develop and implement treatment plans in collaboration with interdisciplinary teams
  • Conduct discharge planning and coordinate community resources
  • Collaborate with physicians, nurses, and other healthcare professionals to ensure holistic patient care
  • Coordinate referrals to community resources, support services, and specialty care
  • Facilitate communication between patients, families, and medical teams
  • Participate in interdisciplinary team meetings and case conferences
  • Provide emotional support and counseling to patients facing medical diagnoses
  • Educate patients and families about medical conditions, treatment options, and coping strategies
  • Assist with advance directive planning and end-of-life care discussions
  • Maintain accurate and timely clinical documentation in proprietary technology and/or electronic health records
  • Ensure compliance with HIPAA, state regulations, and hospital policies
  • Complete required assessments within established timeframes

Requirements

  • Master's degree in Social Work (MSW) from a CSWE-accredited program
  • Current Arizona state licensure as a Licensed Clinical Social Worker (LCSW) or equivalent
  • Valid state professional license in good standing
  • Minimum two years of clinical social work experience preferred
  • Experience in healthcare/medical settings strongly preferred
  • Knowledge of medical terminology and healthcare systems
  • Experience with electronic health record systems
  • Strong assessment and clinical intervention skills
  • Excellent verbal and written communication abilities
  • Knowledge of community resources and referral processes
  • Cultural competency and sensitivity to diverse populations
  • Ability to work effectively in fast-paced, high-stress environments
  • Availability to work standard business hours (9 AM - 5 PM) in Pacific Time
  • Previous experience in healthcare settings with a focus on Cardiology
