Harbor is seeking a full-time Psychiatric LPN or RN to join our Perrysburg team. This position works with other team members in the assessment, diagnosis, and treatment of clients receiving medical services. The role is full-time on the 1st shift.
Requirements
- Associate’s degree from an accredited school of Nursing with a current LPN or RN license in Ohio.
- Two years’ experience working in psychiatric nursing preferred.
- Proficient and accurate in computer use, including Microsoft Word.
- CPR/BLS and NCI/CPI Certifications required within 90 days of employment.
- Valid driver’s license, acceptable driving record, and proof of personal auto insurance policy to be deemed insurable with Harbor’s vehicle insurance carrier.
Responsibilities
- Work at all Harbor sites with a primary site of responsibility, including:
- Reviewing and updating treatment plans for clients receiving pharmacologic management services.
- Ordering and monitoring Central Pharmacy medications.
- Coordinating insurance forms and pre-certifications as necessary.
- Ordering, reordering, or discontinuing prescribed medication into the medication module.
- Completing quarterly checks of first aid boxes, bloodborne pathogen kits, and maintenance of medical equipment.
- Provide medication management of clients through:
- Medication management appointments.
- In-person, phone, or telehealth dispensing of medication samples or Central Pharmacy medications.
- Collaborating with other medical and clinical team members on an ongoing basis.
- Manage medication rooms, maintain records of medication use, and dispose of expired medications.
- Assist other clinical staff with crisis intervention and pre-hospital screening when necessary.
- Provide referral/linkage of clients to appropriate resources.
- Complete the required number of peer review forms.
- Complete all necessary documentation in a timely manner, using formats acceptable to Harbor.
- Support providers by initiating lab requests and coordinating completion of patient medical documentation requests.
- Fulfill or exceed unit expectations for billable services established by Harbor.
- Attend required meetings and serve on committees as requested.
- Provide backup at other sites as needed and requested by supervisor.
- Maintain certification for crisis intervention and pre-hospital screening.
- Participate in professional meetings, workshops, seminars, and continuing education programs to maintain licensure and professional growth.
- May float to other locations to accommodate client or agency needs.
