About the role
The LPN will provide nursing care to clients in outpatient mental health and substance use treatment settings while utilizing the nursing process.
Responsibilities
- Provide direct nursing care to patients with mental health and substance use disorders in an outpatient setting.
- Conduct patient intake, including vital signs, medical/psychiatric history updates, and screening assessments.
- Auxiliary with psychiatric evaluations, procedures, and medication management visits.
- Administer medications as ordered, including oral and injectable treatments, and monitor for side effects or adverse reactions.
- Educate patients on prescribed medications, treatment plans, and symptom management strategies.
- Perform patient triage, including assessing urgency of needs, responding to patient concerns, and coordinating timely care.
- Maintain accurate, timely, and compliant documentation in the electronic health record (EHR).
- Collaborate with providers, supervisor, therapists, case managers, and medical assistants to ensure coordinated, patient-centered care.
- Support crisis intervention efforts using appropriate de-escalation techniques and seek additional clinical support as needed.
- Carefully coordinate care activities such as scheduling, referrals, prior authorizations, and follow-up communication.
- Ensure adherence to all regulatory, safety, and infection control standards, including patient confidentiality requirements.
- Manage workflow, support patient flow, and complete administrative tasks as assigned.
- Utilize information technology systems and software applications effectively and in accordance with organizational standards and security protocols.
- Analyse information thoroughly, reach timely conclusions, and take appropriate, decisive action.
- Plan, prioritize, and organize work to ensure maximum efficiency, productivity, and quality outcomes.
- Solicit and utilize supervision and feedback appropriately to support professional growth and effective job performance.
- Manage time and utilize leave in accordance with organizational policies and procedures.
- Perform additional duties and responsibilities as assigned to support departmental and organizational needs.
- May provide billable services and may be required to meet agency productivity expectations.
- Travel to other locations may be required to meet the operational needs of Harbor.
Requirements
- Current unrestricted LPN license from the state of Ohio.
- CPR/First Aid and NCI/CPI certifications required within 90 days of employment.
- Maintain active licensure in good standing.
- Have a valid driver’s license, acceptable driving record, and show proof of personal auto insurance policy to be deemed insurable with Harbor’s vehicle insurance carrier.
Qualifications
- Education: Current unrestricted LPN license from the state of Ohio.
