Licensed Practical Nurse (LPN)

About the role

Warren Park Health & Living Center is seeking a Licensed Practical Nurse (LPN) to join their team. The LPN will care for a group of residents, assuming responsibility and accountability for them, providing nursing through assessment, implementation, and evaluation of the plan of care.

Responsibilities

• Provide licensed care to assigned residents as ordered by physician and in accordance with facility, federal, state and local standards, guidelines and regulations.
• Perform resident assessments, observations, and interventions; review care plans to ensure appropriate care.
• Take and record residents' blood pressure and vital signs, administer catheter care, IV therapy (within license limitations), and respiratory care such as oxygen and nebulizer treatments.
• Chart nurse’s notes in an informative and descriptive manner reflecting care provided and resident response.
• Ensure the resident’s environment is free from accidents and hazards; immediately correct or address potential safety risks.
• Monitor and direct non‑licensed ancillary services as needed to maintain professional standards and high‑quality resident care.
• Communicate and report information to residents, families, physicians, guardians, governmental agencies, and facility personnel as required by policy and regulations.
• Implement and maintain all aspects of resident care plans; make recommendations for care plan changes to the interdisciplinary team.
• Ensure compliance with infection control standards and promptly address any non‑compliance.
• Follow HIPAA policies and procedures.
• Perform all other duties as assigned.

Qualifications

• Current Licensed Practical Nurse license in the state.
• Nursing degree or diploma from an accredited college or university.
• One year of experience preferred; long‑term care experience preferred.
• Excellent written and verbal communication skills in English.
• Ability to multi‑task and remain calm under stress.
• Analytical and problem‑solving skills.
• Understanding and adherence to infection control standards, policies, and procedures.
• Ability to adapt to a flexible work schedule and occasional overnight travel.
• Ability to relate positively and effectively with residents, families, community members, volunteers, and staff.

Physical Requirements

Walking, reaching, bending, lifting, extended sitting, grasping, fine hand coordination, pushing and pulling, and the ability to distinguish smells and temperatures, with or without mechanical assistance. Limited potential for exposure to environmental hazards. Understanding and adherence to company safety standards and protocols required.
