Program Supervisor - Ludden
About the Role
The Program Manager directly supervises Direct Support Professional staff, ensures proper implementation of The Phoenix Residence policies and procedures, and oversees the coordination and implementation of Coordinated Service and Support Plans/Individual Program Plans. This includes programs in activities of daily living, community integration, socialization, and health management. The role involves completing functions as the program's Designated Coordinator and/or working with the designated QIDP to implement and monitor individual program plans. This is an hourly position requiring 40-50 hours every two weeks, including filling in shifts as needed, with the remainder dedicated to managerial duties.
Responsibilities
- Program Operation: Oversee the overall operation of one ICF-DD. Train, supervise, evaluate, and manage House Supervisors and Direct Support Professional staff.
- Ensure 24-hour coverage of the home and monitor services across multiple shifts while maintaining staffing levels according to budgetary and safety guidelines.
- Maintain the appearance of the house and grounds, ensuring standards for sanitation and infection control are met.
- Complete audits to oversee and monitor services.
- Individual Services:
- Perform, oversee, and train direct care duties for individuals living in the home.
- Develop, implement, and monitor individualized Program Plans, including staff training on plan implementation.
- Coordinate and facilitate individual meetings.
- Complete QIDP duties according to regulatory requirements.
- Advocate for the needs of individuals residing within the home.
- Manage individuals' finances per established procedures.
- Handle household operations such as cooking, cleaning, laundry, and ordering groceries/supplies.
- Identify and respond to individual needs in emotional and social areas (excluding psychotherapy).
- Perform tasks within Category II of infection control, which may include exposure to blood, body fluids, or tissue.
- Safety:
- Adhere to all safety policies and procedures, fostering a safe living and working environment.
- Follow established lifting and transfer guidelines and train others in proper techniques.
- Regulation Compliance:
- Understand, implement, and comply with all policies, procedures, and philosophy of The Phoenix Residence, as well as rules and regulations of licensing agencies.
- Maintain working knowledge of rules including Rule 245D, Rule 10, 185, Positive Support Rule, Supervised Living Facility Regulations, Life Safety Code, Food and Beverage regulations, funding rules, and employment laws.
- Fiscal Integrity: Participate in fundraising efforts such as annual events, mass mailings, capital campaigns, planned giving, and grant writing to ensure the fiscal integrity of the organization.
- Development of Position:
- Attend all mandatory in-services and meet annual training requirements.
- Take on extra projects and participate in at least one committee.
- Customer/Human Relations:
- Communicate effectively with individuals, staff, supervisors, families, interdisciplinary team members, and other customers verbally and in writing.
- Display cooperative, supportive, and positive conduct while serving as an effective role model.
Requirements
- High School Diploma (required).
- Minimum of 2 years of supervisory/management experience or a four-year BA/BS degree in a related field (e.g., sociology, psychology, social services).
- Ability to assist with Qualified Intellectual Disabilities Professional (QIDP) duties; QIDP certification preferred.
- CPR, First Aid, and Trained Medication Passer certification within the first 60 days of employment.
- Valid driver’s license and a safe driving record; trained to drive Phoenix-owned vehicles within the first 60 days of employment.
- Ability to maintain a flexible schedule, including 44 hours of direct support care per pay period, weekends, and holidays.
- Responsibility for 24-hour on-call coverage and responding to issues as necessary.
- Excellent problem-solving, decision-making, and organizational skills.
- Proficiency in reading, writing, and speaking English to respond to individuals' needs.
- Effective verbal and written communication skills; ability to listen accurately and respond in a timely manner.
- Ability to see and hear to make assessments.
- Ability to frequently lift up to 75 pounds, including lifting objects and/or transferring individuals.
- Ability to sit, stand, stoop, bend, reach, and push/pull wheelchairs.
- Ability to respond to emergencies in accordance with procedures and guidelines.
- Proficient in Microsoft Office (Word, Excel, Outlook) and ability to use a computer for documentation, communication, timekeeping, and assessments.
- Ability to operate and maintain household, office, and facility equipment.
- Ability to be employed full-time.
Schedule
This position requires 40-50 hours every two weeks, including 44 hours of direct support care per pay period. The schedule is flexible and may include working weekends and holidays. The role also involves 24-hour on-call coverage.
