Responsibilities

  • Coordinate Services
    • Ensure access to community services/resources, opportunities for education, social, recreational, and employment goals, and connections with others.
    • Support staff in assisting program participants to develop tools necessary for achieving goals such as employment, preferred environments, and connections with others.
    • Provide other types of support and instruction to persons served as requested.
  • Develop/Implement Service Plans and Write Reports
    • Facilitate person-centered planning processes and develop Individual Service Plans (ISPs) based on skills, abilities, and interests.
    • Coordinate staff activities to ensure adherence to A Bright Future, Inc. and funding/regulatory agency expectations regarding data collection, progress reporting, and report submissions.
    • Maintain files for persons served in accordance with A Bright Future, Inc. policies.
    • Report all suspected abuse in accordance with mandated reporter regulations and adhere to A Bright Future, Inc. expectations for reporting special incidents.
  • Create Community Connections
    • Seek opportunities to collaborate with employers and community/service organizations.
    • Facilitate and assist individuals in developing a circle of support that includes family, friends, neighbors, and community members.
    • Provide education and support to community members and assist individuals in negotiating support needs.
  • Develop and Maintain Effective Relationships
    • Display a professional and respectful attitude towards individuals served, coworkers, supervisors, community employers, and other agency affiliates.
    • Work effectively as part of the A Bright Future, Inc. team to accomplish the mission of the organization.
    • Model person-centered thinking philosophies and promote individuals served being at the center of decision-making.
    • Solve issues related to individuals served through a team approach.
    • Promote continuous quality service improvement and teamwork among co-workers and individuals served.
  • Other Duties
    • Perform other duties as requested.

Requirements

  • 21 years of age or older
  • High School diploma or equivalent required; Bachelor’s degree in social services preferred
  • Three (3) years’ experience working in a human services related field; and the demonstrated ability to provide staff training, supervision, and planning
  • One (1) year minimum experience coordinating services for individuals with disabilities preferred
  • Experience in job/task analysis, work assessment, teaching techniques or equivalent experience is desirable
  • Valid CA driver’s license, acceptable driving record, and current auto insurance for a car used during work time as needed
  • Fingerprint/Criminal record clearance with a Child Abuse Index check from the Department of Justice prior to employment
  • Physical examination and tuberculosis test prior to employment
  • Ability to work a flexible schedule which may include nights and weekends
  • Ability to self-direct daily work activities with little supervision
  • Ability to maintain an orderly work environment, prioritize workload, and perform tasks in a prescribed and safe manner
  • Demonstrated sensitivity to the multi-cultural diversity of the service population
  • Willingness to enroll in ongoing, free training to expand and develop skills
  • Experience with Windows operating system, Microsoft Office, and database reporting
  • First Aid and CPR certifications are required

Qualifications

Salary, Non-Exempt

Schedule

Monday-Friday; 9am-5:30pm - occasional overtime + occasional weekends
