Program Manager, Targeted Case Management
MHP is hiring a Program Manager to provide overall service coordination of a Targeted Case Management team that delivers flexible, community-based peer support to individuals with co-occurring disorders and those experiencing homelessness. These services promote stability, recovery, and community integration for individuals with mental health conditions, substance use, and potential physical health concerns.
About the role
The Program Manager coordinates services for assigned programs, mentors staff, and ensures participants' self-identified goals for recovery, rehabilitation, and community integration are supported through appropriate services and best practices. This is a full-time position located at 2709 N Broad St, Philadelphia, PA 19132.
Responsibilities
- Leads overall service coordination in alignment with MHP and regulatory guidelines; ensures adherence to accreditation, certification, and regulatory standards; establishes and implements operational procedures and monitors participant visits.
- Hires, trains, evaluates, and develops staff in support of MHP and Recovery core policies, practices, and principles; facilitates and documents individual supervision every other week and daily morning team meetings.
- Manages program staff schedules.
- Cultivates and maintains working relationships with outside agencies, including behavioral health organizations, healthcare providers, and community organizations; attends professional meetings as required.
- Reviews and approves employees' electronic health record entries; ensures participant satisfaction through service verifications, auditing, and monitoring records.
- Assists participants with mental health and substance abuse services, acquiring stable housing, and other participant-identified goals in accordance with their support plan; ensures continuity of care.
- Tracks revenue from mobile services and incorporates individual production reports into staff supervision sessions to support staff in achieving billable unit quotas.
- Defuses critical situations and provides safety and reassurance to participants, volunteers, and staff; notifies the Division Director of any problems or concerns; creates incident reports as necessary.
- Gathers and provides data for outcomes and written reports requested by city, state, or federal funding sources.
- May cover Targeted Case Management responsibilities to maintain program operations and ensure continuity of care for participants.
- Performs other duties as assigned to fulfill MHP's mission.
Requirements
- Master’s degree in a human services field required.
- 2 years of direct care experience in mental health.
- 3 years of management/supervision experience (including managing functions, processes, and/or people in work, internship, or volunteer settings).
- Knowledge of clinical theory and practice, community resources and systems, financial/social resources, and experience in interviewing, assessment, and implementation.
- Basic computer skills for desktop computing, email, timesheet management, electronic health records, and internet research.
- Ability to work under pressure, meet deadlines, and maintain a positive attitude while providing exemplary customer service.
- Ability to work independently and complete assignments within prescribed parameters.
- Ability to understand and effectively manage a program budget.
- Strong oral and written communication, cultural competence, interpersonal relationships, teamwork, attention to detail, time management, autonomy, problem-solving, and organizational skills.
- Valid driver’s license, access to a car for daily work use, and automobile insurance coverage at state-required levels with current registration.

Physical Demands
- Sit at a desk with ergonomically appropriate equipment.

- Ability to maintain regular, punctual attendance and be available for a flexible work schedule, including evenings, weekends, and holidays/overtime as needed.
- Travel within the community, which may not be handicap accessible; walking and/or driving agency vehicles, using public transportation, moving around inner-city neighborhoods daily (including in inclement weather), carrying computer equipment, and ascending/descending stairs.
- Frequent talking and listening; must communicate clearly and accurately, with possible exposure to loud or vulgar language and hostile gestures.
- If part of an essential service, may be required to work on holidays or during emergency closings.

Pay
$56,000 – $61,600 annually.
Schedule
- 40 hours per week, Monday – Friday, 8:30 AM – 5:00 PM.
- Option to work from home once a week after successful onboarding and based on program needs.
