Program Manager - Community Living DeSoto

About the role

Pony Bird is looking for a qualified, compassionate Program Manager with experience working with individuals with disabilities to provide care and coordination of service. We provide comprehensive care for individuals with intellectual and physical disabilities in an enriching, challenging, and supportive environment.

Responsibilities

• Develop and implement Individual Service Plans, based on assessments and observations.
• Responsible for 24-hour-a-day Group Home with rotating on-call and scheduling.
• Supervise, plan, assign, and evaluate or monitor programming and the work of staff responsible for the daily care of individuals.
• Work closely with therapists, service coordinator, administrations, and parents/guardians to maintain communication and provide feedback, standardized procedures, expedite support plan implementation and workflow, and improve the efficiency of all care/support providers.
• Perform various assessments; chart reviews; documentation, mealtime and environmental monitoring.

Requirements

• BS in Social Work, Social Services, Nursing, Special Education, or related field from an accredited four-year college or university.
• 2 or more years' experience in Program Management in a similar setting.
• Proficient in MS Office suites and various industry software programs.
• Valid Missouri License with an insurable driving record.
