Overview

Key Rehab is seeking a Full Time, Program Coordinator (OT or PT) to join their team in the Humboldt area at a SNF setting. Apply now to receive a Sign-on Bonus up to $5,000**.

Education

  • Required: Completion of an accredited PT, OT, or SLP program
  • Preferred: Prior experience in long-term care setting and service coordination

Licenses/Certificates

  • Required: Current PT, OT, or SLP license to practice in the state(s) assigned
  • Required: Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP)
  • Required: CPR, as required by community
  • Required: Valid driver license and car insurance as required by state law
  • Required: Other items as required by federal, state and local standards

Responsibilities

  • Provide therapy under their disciplinary scope of practice (OT, PT, or SLP)
  • Serve as the liaison between Key Rehab and community staff
  • Minimize administrative burden and provide support to allow clinicians to deliver therapy services
  • Cookstaff and daily scheduling to meet key performance indicators and expectations under the direction of a Regional Supervisor

  • Flexibility and Team Collaboration: Adaptable to changing situations and committed to working seamlessly within a team environment
  • Exceptional Communication: Outstanding oral and written communication skills for clear and effective interaction with patients and colleagues
  • Experience with Geriatric Population: Prior experience working with elderly patients, demonstrating an understanding of their unique needs
  • Knowledge of Medicare Billing Regulations: Familiarity with current Medicare billing practices and guidelines
