Professional I, Behavioral Health Specialist; Permanent Supportive Housing

The Centers offers a $1,200 sign-on bonus for this full-time, non-exempt position supporting operations across multiple locations in the Cleveland area.

About the Role

This position supports our Permanent Supportive Housing (PSH) program, which combines low-barrier, affordable housing with integrated health care and wraparound supportive services. As a Case Manager, you will help individuals and families who have experienced chronic homelessness build stability through strength-based case management and whole-person care.

Responsibilities

• Build and sustain therapeutic relationships with clients through face-to-face, telephonic, office-based, property-based, and community-based engagement.
• Conduct comprehensive assessments of strengths and needs, administer screening tools, and support clients in developing independent living skills (e.g., budgeting, lease understanding, bill payment).
• Develop and maintain individualized service plans and housing success plans using evidence-based strategies (Housing First, Harm Reduction, Motivational Interviewing, Stages of Change).
• Provide life skills training and pro-social support, individually and in groups, and monitor progress to adjust interventions.
• Coordinate with a multidisciplinary team (counselors, nurses, physicians, pharmacists, social service staff, Property Management) to connect clients to community resources (primary care, behavioral health, substance use services, benefit programs, vocational resources).
• Assist clients with benefit applications, advocacy, appeals, and transitions in care following hospitalizations or disruptions.
• Recognize behavioral health, physical health, and substance misuse presentations; apply risk and lethality assessments; and use clinical supervision effectively.
• Document all case management activity in the electronic medical record system, ensuring clarity, objectivity, and compliance with privacy laws.
• Participate in multidisciplinary team meetings, housing inspections, move-in/move-out planning, and community-building activities.
• Support onsite food pantry operations approximately once per month and maintain required food safety certification.

Requirements

• High school diploma or equivalent (required). Bachelor’s degree in Social Work, Psychology, or a mental health-related field (preferred).
• At least three years of case management or care coordination experience (required). Prior experience with adults with serious and persistent mental illness (preferred).
• Valid Ohio driver’s license and access to an insured vehicle for client transportation and community-based services.
• Nonviolent Crisis Intervention certification (required within 60 days of hire). Active Community Health Worker certification (preferred).

Skills

• Ability to work collaboratively in a multidisciplinary team and develop therapeutic relationships with clients.
• Working knowledge of behavioral health, substance use, trauma-informed care, harm reduction, and housing stability practices.
• Proficiency in evidence-based practices (e.g., Motivational Interviewing, Stages of Change).
• Strong documentation skills and adherence to client privacy and confidentiality laws.

Working Conditions

• Operates in professional office, property-based, and community-based environments across the Cleveland area.
• Local travel required for case management, meetings, trainings, client transportation, and community-based services.
• May occasionally lift up to 40 pounds for site, delivery, or food pantry activities.
• Application of approved nonviolent crisis intervention and de-escalation techniques when needed.
