PRN Physical Therapist (PT)

About the role

Enhance Rehabilitation is seeking qualified Physical Therapists to join our dynamic team, providing the highest quality care for patients in a Skilled Nursing Facility (SNF) setting.

In this role, you will evaluate and treat patients following an individualized plan of care, ensuring excellent care and customer service to patients and their family members.

Responsibilities

• Collaborate with facility staff to ensure excellent care for patients.
• Evaluate new patients, develop plans of care, and set measurable, functional goals.
• Treat patients, address their concerns, and educate them on their injury, progress, and probable outcomes.
• Supervise Physical Therapist Assistants.
• Re-evaluate patient progress, revise plans of care, and reset goals as needed.
• Complete timely and accurate documentation, including evaluations, re-evaluations, discharges, and daily progress notes.
