Responsibilities
- Evaluate residents and develop individualized treatment plans
- Provide skilled physical therapy services focused on mobility, strength, balance, and functional independence
- Accurately document evaluations, treatments, and progress in Net Health and PCC
- Collaborate with nursing, therapy, and interdisciplinary teams
- Maintain compliance with facility and regulatory standards
Qualifications
- Current Ohio PT license
- SNF/LTC experience preferred but not required
- Experience with Net Health and PCC preferred
- Strong communication and clinical skills
- Compassionate, dependable, and team-oriented mindset
