Join our dynamic team as a Physical Therapist in a Skilled Nursing Facility (SNF) setting, delivering the highest quality care to patients through individualized treatment plans.

Responsibilities

• Evaluate and treat patients following an individualized plan of care.
• Provide excellent care and customer service to patients and their family members.
• Collaborate with facility staff to ensure optimal patient outcomes.
• Conduct evaluations for new patients, develop plans of care, and set measurable, functional goals.
• Treat patients, address their concerns, and educate them on their injury, progress, and probable outcomes.
• Supervise Physical Therapist Assistants.
• Re-evaluate patient progress, revise plans of care, and reset goals as needed.
• Complete timely and accurate documentation, including evaluations, re-evaluations, discharges, and daily progress notes.
