Responsibilities
- Evaluate patients' functional status and develop personalized treatment plans
- Perform interventions to improve mobility, relieve pain, and restore or maintain function
- Assist physicians in creating therapy plans and coordinate with healthcare team members
- Monitor and report patient responses and progress
- Educate patients, families, and caregivers on care plans and functional goals
- Complete patient evaluations, progress notes, and ensure compliance with guidelines
Requirements
- Current Physical Therapy licensure in the practice state
- CPR certification
- Valid driver’s license, reliable transportation, or access to public transit
- Ability to adapt to diverse home environments and serve patients regardless of background
