PRN - Physical Therapist

Responsibilities

• Evaluate patients' functional status, mobility, endurance, pain, and medical conditions.
• Assist in developing and implementing individualized treatment plans.
• Perform interventions to improve patient well-being and restore or maintain function.
• Monitor and report patient responses and progress to the healthcare team.
• Educate patients, families, and team members on care plans and progress.
• Complete patient evaluations, progress notes, and reports according to guidelines.
• Conduct assessments, develop treatment plans, deliver therapies, document progress, and collaborate with the healthcare team to enhance patient outcomes.

Requirements

• Current Physical Therapy license in the practice state.
• CPR certification and valid driver's license with access to transportation.
• Ability to work effectively in diverse home environments.

Pay
