PRN - Physical Therapist

About the Role

Provide outpatient physical therapy services in home health settings, focusing on assessments, treatment planning, interventions, and progress evaluation.

Responsibilities

• Evaluate patients' functional status and develop treatment plans in collaboration with physicians
• Deliver therapy within legal scope and coordinate with healthcare team
• Monitor and report patient responses and condition changes
• Treat patients to improve function, relieve pain, and maintain maximum capability
• Educate patients, families, and team members about care plans and progress
• Complete evaluations, progress notes, and reports according to policies and guidelines
• Document outcomes, goals, and projected care frequency

Requirements

• Current state licensure
• CPR certification
• Valid driver’s license
• Dependable transportation
• Ability to work in diverse home environments
• Strong communication, assessment, and treatment skills
