Responsibilities
- Complete assessments and evaluations of patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions.
- Develop and implement a treatment plan based on patient assessments and evaluations to improve and enhance the patient's well-being.
- Evaluate patient progress and adjust treatment plans as needed.
- Assist the physician in developing the therapy plan of care.
- Provide services within the scope of practice as defined by state laws, in accordance with the plan of care, and in coordination with other members of the health care team.
- Observe and report the patient's response to treatment and any changes in condition to the supervising nurse and/or physician.
- Treat patients to relieve pain, develop or restore function, and maintain maximum functioning using acceptable standards of practice.
- Instruct and inform the health care team, patient, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
- Visit patients according to the Plan of Treatment, complete a progress note for each visit, and submit progress notes per policy.
- Complete all patient evaluations and develop the physical therapy plan of care within state-specific guidelines.
- Report outcomes of evaluation, goals, and anticipated projected frequency of care.
Requirements
- Current Physical Therapy licensure in the state of practice.
- Current CPR certification.
- Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
- Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
Pay
Annual target pay: $82,048 - $123,072 (assumes full-time employment at 40 weekly hours and full productivity).
- Hourly pay range: $39.45 - $59.17 (for non-visit activities such as orientation).
- Per visit point pay range: $47.34 - $71.00 (for patient visits, calculated by multiplying the per visit point rate by productivity points accrued).
Total cash compensation includes earnings from per visit point pay and hourly pay, and may vary based on local labor markets, education, work experience, productivity, and performance.
