Responsibilities

  • Patient Assessment: Evaluate patients' and families' psychosocial, emotional, and environmental needs, assessing their ability to cope with illness, injury, or life transitions in the home setting.
  • Treatment and Education: Provide supportive counseling, crisis intervention, and connect patients with vital community resources (such as financial assistance, long-term care planning, home modifications, and local support networks). Educate patients and families on coping strategies and available health and social services.
  • Documentation: Maintain accurate and timely records of evaluations, progress notes, and care plans, ensuring compliance with legal, regulatory, and ethical standards.
  • Collaboration: Work closely with physical therapists, nurses, physicians, and other healthcare providers to ensure a holistic approach to patient care and participate in multidisciplinary team meetings as needed.

Requirements

  • Education: Master of Social Work (MSW) degree from an accredited institution.
  • Licensure: Valid State Licensure as a Social Worker (LMSW, LCSW, LMHP, or LISW) in Nebraska and/or Iowa, or license eligible.

Skills

  • Strong clinical assessment and problem-solving skills.
  • Proficient in developing and implementing effective psychosocial treatment plans.
  • Excellent communication and interpersonal skills with a patient-centered approach.
  • Reliable transportation for home visits throughout the metro area.

Physical Requirements

  • Ability to travel to patient homes in various weather conditions.
  • Must be able to sit or stand for periods and perform standard office and field tasks related to social work.
