Home Health Social Worker PRN

About the role

The Medical Social Worker collaborates within an interdisciplinary care team to evaluate and develop personalized care plans addressing patients' emotional and social needs in a home health setting.

Responsibilities

• Assess patients' social, emotional, and medical needs, including relationships and community resources.
• Facilitate access to community resources to support recovery and well-being.
• Communicate effectively with patients, families, and healthcare team members.
• Provide ongoing assessments of patient and family responses to treatment and coping strategies.
• Participate in care coordination and act as a resource for team members.
• Supervise and evaluate the Social Work Assistant to ensure compliance with standards.
• Maintain thorough documentation of patient interactions, care plans, and progress.

Qualifications

• Minimum one year of healthcare social work experience, including home health or hospice.
• State licensure as a social worker, if required.
• Valid driver’s license, reliable transportation, and proof of CPR certification.
• Experience in discharge planning and community resource navigation.
• Knowledge of medication administration and healthcare regulations.
• Strong organizational skills, ability to prioritize, and work under stress.
• Fluent in English and adaptable to flexible work hours and travel.
