About the role

The Medical Social Worker collaborates with the interdisciplinary care team to assess and address patients' social and emotional needs in a home health setting. They evaluate patient relationships, financial resources, and community support options, developing personalized care plans to promote recovery and well-being.

Responsibilities

Responsibilities include providing ongoing assessments, coordinating resources, communicating with patients and families, supervising the Social Worker Assistant, and maintaining documentation in compliance with policies and regulations.

Qualifications

• Minimum one year of healthcare social work experience, including home health or hospice
• State licensure as a social worker, if applicable
• Valid driver’s license, auto insurance, and reliable transportation
• Current CPR certification

Skills

• Experience with discharge planning and community resource navigation
• Knowledge of medication administration and ability to organize tasks under stress
• Strong communication skills and flexibility in work hours and travel
• Fluent in English; maintains licensure and continuing education requirements
