About the role

The Medical Social Worker plays a vital role in providing personalized emotional and social support to home health patients. They assess patients' social and emotional needs related to their illness, evaluate relationships, financial resources, and community support options. The position involves coordinating with the interdisciplinary care team, guiding patients and families through care plans, and managing discharge planning and resource connection.

Responsibilities

• Assess patients' social and emotional needs related to their illness.
• Evaluate patients' relationships, financial resources, and community support options.
• Coordinate with the interdisciplinary care team to support patient care plans.
• Guide patients and families through care plans and discharge planning.
• Connect patients with community resources and support services.
• Supervise the Social Worker Assistant.
• Maintain accurate and timely documentation in compliance with healthcare standards, policies, and regulations.

Requirements

• At least one year of healthcare social work experience.
• Valid social work licensure.
• Valid driver’s license.
• CPR certification.
• Knowledge of community resources and medication management.
• Flexibility for local travel.
• Ongoing licensure maintenance.
• Fluency in English.
