Responsibilities
- Providing skilled nursing care according to care plans and physician orders
- Assessing patient conditions, monitoring changes, and reporting to supervisors or physicians
- Documenting all nursing activities and responses accurately and timely
- Developing, implementing, and updating individualized care plans
- Educating patients and families on disease processes, self-care, and prevention
- Maintaining infection control standards and complying with policies and regulations
- Participating in ongoing education to enhance nursing competencies
Qualifications
- Unrestricted, active RN license in the practicing state
- Current CPR certification
- Graduate of an accredited nursing program
- Reliable transportation and ability to travel
- Preferred: recent clinical experience (minimum 6 months), home health experience
Skills And Abilities
- Attention to detail and strong organizational skills
- Effective communication and problem-solving abilities
- Physical ability to lift up to 50 pounds, perform mobility tasks, and respond to emergencies
Additional Requirements
- Proficiency in assessments and documentation
- Ability to work in diverse environments, possibly exposed to infectious materials
- Compliance with health and safety standards, including vaccinations where applicable
