Private Duty Nurse LVN - Nights

About the role

We are seeking a Licensed Practical / Vocational Nurse (LPN/LVN) to provide skilled nursing care in a home-based setting. This role emphasizes professionalism, adaptability across various environments, and commitment to patient well-being.

Responsibilities

• Deliver and document personalized patient care according to physician orders and care plans.
• Monitor patient conditions, recognize changes, and report to supervisors or physicians.
• Educate patients and families on disease management, self-care, and prevention.
• Maintain accurate documentation of assessments, interventions, and responses.
• Adhere to infection control standards, confidentiality, and professional boundaries.
• Participate in ongoing education and skill development.
• Provide high-quality, compassionate care tailored to individual patient needs.
• Assess and respond to patient condition changes.
• Ensure documentation accuracy and compliance with regulations.
• Support patient education and safety during care delivery.

Requirements

• Active, unencumbered LPN/LVN license in the practice state.
• Current CPR certification and TB test within the past 12 months.
• Preferred six months of clinical nursing experience; home health experience is a plus.
• Reliable transportation.
• Attention to detail, strong communication, and problem-solving skills.
• Physical ability to lift up to 50 pounds, reposition patients, and perform home care tasks.
