About the role
We are seeking a Licensed Practical/Vocational Nurse (LPN/LVN) to provide skilled nursing care in patients' homes, under the supervision of a Registered Nurse.
Responsibilities
- Deliver and coordinate quality patient care
- Assess patient conditions
- Document activities and update care plans
- Educate patients and families
- Maintain confidentiality and adhere to policies and regulations
- Practice infection control
- Respond promptly to patient needs
Requirements
- Active, unencumbered nursing license
- Current CPR certification
- TB test within the last 12 months
- Reliable transportation
Qualifications
Preferred candidates have at least six months of clinical experience or a home health background.
