Role Overview
The Licensed Practical/Vocational Nurse (LPN/LVN) provides skilled nursing care under supervision, following care plans and physician orders.
Responsibilities
- Deliver and coordinate quality patient care
- Assess and report patient condition changes
- Document activities accurately
- Update care plans
- Implement interventions to meet patient needs
- Ensure infection control
- Educate patients and families
- Adhere to professional and regulatory standards
Qualifications & Skills
- Active, unencumbered LPN/LVN license in the practicing state
- Current CPR certification
- TB test within the last 12 months
- Six months of recent clinical experience (preferred)
- Home health experience (a plus)
- Attention to detail

Physical & Environment Requirements
- Ability to lift 50 pounds
- Ability to reposition patients
- Ability to travel
