About the Role

Join a dynamic healthcare team as a Licensed Practical / Vocational Nurse (LPN/LVN). Responsibilities include providing and documenting skilled nursing care under RN supervision, following care plans and physician orders.

Responsibilities

• Assess patient conditions and report changes
• Update care plans and administer appropriate interventions
• Educate patients and families on self-care and disease management
• Ensure compliance with confidentiality, infection control, and regulatory standards

Requirements

• Active, unencumbered LPN/LVN license in the practicing state
• Current CPR certification and TB test within the last 12 months
• Six months of clinical nursing experience preferred
• Reliable transportation
