Private Duty Nurse LVN - Full Time

About the Role

We are seeking a Licensed Practical / Vocational Nurse (LPN/LVN) to provide skilled nursing care in patients' homes, following physician orders and care plans.

Responsibilities

• Deliver and coordinate quality patient care while ensuring compliance with regulations.
• Monitor patient conditions, documenting assessments, interventions, and responses accurately and timely.
• Implement updated nursing care plans and take appropriate actions based on patient assessments.
• Recognize and respond to changes in patient needs to prevent or reduce risks.
• Educate patients and families on disease management, self-care, and prevention strategies.
• Maintain infection control standards and adhere to confidentiality and professional boundaries.

Qualifications

• Active, unencumbered LPN/LVN license in the practice state.
• Current CPR certification and TB test within the last 12 months.
• Six months of hands-on nursing experience preferred but not required.
• Reliable transportation and proficiency in clinical assessments, documentation, and compliance.

Skills and Physical Requirements

• Attention to detail, time management, problem-solving, and effective communication.
• Ability to lift 50 pounds, reposition patients, and respond appropriately to emergencies.
• Proficiency in English, ability to travel, and physical capacity for prolonged mobility and environmental adaptability.

Work Environment

This role involves working in diverse home environments, possibly exposing staff to infectious diseases and allergens, with duties subject to change.
