Private Duty Nurse LVN

About the role

We are seeking a Licensed Practical/Vocational Nurse (LPN/LVN) to provide skilled nursing care in patients' homes, under the supervision of a Registered Nurse.

Responsibilities

• Delivering and documenting patient care according to care plans and physician orders.
• Assessing patient conditions, recognizing changes, and reporting to supervisors or physicians.
• Implementing nursing actions to achieve expected outcomes and prevent risks.
• Educating patients and families on disease management, self-care, and prevention.
• Maintaining compliance with policies, regulations, and infection control standards.
• Participating in ongoing education and skill development.

Qualifications and skills

• Current, active LPN/LVN license in the practicing state.
• CPR certification and recent clinical assessment experience preferred.
• Reliable transportation, attention to detail, good communication, and problem-solving skills.
• Physical ability to lift up to 50 pounds, reposition patients, and handle home environments.

This role requires adaptability to diverse environments and adherence to health protocols, including exposure to bodily fluids and infectious agents.
