Private Duty Nurse LVN

About the role

We are seeking a Licensed Practical / Vocational Nurse (LPN/LVN) to provide skilled nursing care in patients' homes under supervision.

Responsibilities

• Deliver and coordinate patient care per physician orders, ensuring adherence to care plans.
• Assess patient conditions, document activities, and report changes promptly.
• Implement nursing actions to achieve desired outcomes and prevent risks.
• Educate patients and families on disease management, self-care, and prevention.
• Maintain infection control standards and comply with relevant regulations and policies.
• Participate in ongoing education and skill development.

Requirements

• Active, unrestricted LPN/LVN license in the practicing state.
• Current CPR certification and TB test within the last 12 months.
• Reliable transportation and ability to perform physical tasks such as lifting and repositioning patients.
• Strong attention to detail, organization, communication, and problem-solving skills.
• Ability to operate in diverse environments, including exposure to infectious materials.

Qualifications

• Preferred six months of recent clinical experience; home health experience is a plus.
• Physical ability to handle the demands of homecare, including mobility and emergency response.
